Position:  Transition Coordinator

 

Job Summary:

To plan, guide, and evaluate the vocational and transition process for students with special needs.

 

  • Oversee Indicator 13 – maintain current understanding of state and federal transition requirements. 

  • Provide professional development to all secondary teachers (middle & high school) regarding Indicator 13 requirements.  

  • Attend case conferences.

  • Coordinate the development and implementation of Transition Portfolios, requirements set forth by IDOE. 

  • Work with families on how to apply for the Medicaid Waiver. Help families oversee this process. 

  • Assure opportunity for students and parents to connect with the Vocational Rehabilitation Counselor, when appropriate. 

  • Solicit local businesses to accept job placements for students. 

  • Coordinate appropriate assessment and placement for students into vocational settings, troubleshoot concerns, and conduct follow-up and evaluation of placement. 

  • Attend meetings for the Transition Cadre.

Qualifications:     

  • Desire and ability to work with students with diverse backgrounds and levels of ability toward accomplishing their educational and post-secondary goals.

  • Ability to organize tasks and manage time. 

  • Communicate effectively with students, parents, school administrators and other staff.
